Calcium gluconate is a mineral supplement and medication.[1] As a medication it is used by injection into a vein to treat low blood calcium, high blood potassium, and magnesium toxicity.[2][1]
Medical uses[edit]Low blood calcium[edit]10% calcium gluconate solution (given intravenously) is the form of calcium most widely used in the treatment of low blood calcium. This form of calcium is not as well absorbed as calcium lactate,[9] and it only contains 0.93% (930 mg/dl) calcium ion (defined by 1 g weight solute in 100 ml of solution to make 1% solution w/v). Therefore, if the hypocalcemia is acute and severe, calcium chloride is given instead.
High blood potassium[edit]Calcium gluconate is used as a cardioprotective agent in people with high blood potassium levels, with one alternative being the use of calcium chloride.[10] It is recommended when the potassium levels are high (>6.5 mmol/l) or when the electrocardiogram (ECG) shows changes due to high blood potassium.[2]
Though it does not have an effect on potassium levels in the blood, it reduces the excitability of cardiomyocytes, thereby lowering the likelihood of cardiac arrhythmias.[11]
Magnesium sulfate overdose[edit]It is also used to counteract an overdose of Epsom salts magnesium sulfate,[12] which is often administered to pregnant women in order to prophylactically prevent seizures (as in a patient experiencing preeclampsia). Magnesium sulfate is no longer given to pregnant women who are experiencing premature labor in order to slow or stop their contractions (other tocolytics are now used instead due to better efficacy and side effect profiles).[citation needed] Excess magnesium sulfate results in magnesium sulfate toxicity, which results in both respiratory depression and a loss of deep tendon reflexes (hyporeflexia).
Hydrofluoric acid burns[edit]A tube of calcium gluconate gelGel preparations of calcium gluconate are used to treat hydrofluoric acid burns.[13][14] This is because calcium gluconate reacts with hydrofluoric acid to form insoluble, non-toxic calcium fluoride.
Black widow spider bites[edit]Historically, IV calcium gluconate was used as an antidote for black widow spider envenomation, often in conjunction with muscle relaxants.[15] This therapy, however, has since been shown to be ineffective.[16][17]
Cardiac arrest[edit]While intravenous calcium has been used in cardiac arrest its general use is not recommended.[1] Cases of cardiac arrest in which it is still recommended include high blood potassium, low blood calcium such as may occur following blood transfusions, and calcium channel blocker overdose.[1] There is the potential that general use could worsen outcomes.[1] If calcium is used calcium chloride is generally the recommended form.[1]

Calcium lactate has several uses in human and veterinary medicine.
Calcium lactate is used in medicine as an antacid.[citation needed]
Calcium lactate is also used to treat hypocalcaemia (calcium deficiencies). It can be absorbed at various pHs, thus it does not need to be taken with food. However, in this use it has been found to be less convenient than calcium citrate.[12]
In the early 20th century, oral administration of calcium lactate dissolved in water (but not in milk or tablets) was found to be effective in prevention of tetany in humans and dogs with parathyroid insufficiency or who underwent parathyroidectomy.[13][14]
The compound is also found in some over the counter mouth washes.[citation needed]
Calcium lactate (or other calcium salts) is antidote for soluble fluoride ingestion[15]:p165 and hydrofluoric acid.
Food industry[edit]The compound is a food additive classified by the United States FDA as Generally Recognized as Safe (GRAS), for uses as firming agent, a flavor enhancer or flavoring agent, a leavening agent, a nutritional supplement, and a stabilizer and thickener.[16]
Calcium lactate is an ingredient in some baking powders containing sodium acid pyrophosphate. It provides calcium in order to delay leavening.[17]:p933
Calcium lactate is added to sugar-free foods to prevent tooth decay. When added to chewing gum containing xylitol, it increases the remineralization of tooth enamel.[18]
The compound is also added to fresh-cut fruits, such as cantaloupes, to keep them firm and extend their shelf life, without the bitter taste caused by calcium chloride, which can also be used for this purpose.[19]
Calcium lactate is used in molecular gastronomy as a flavorless fat-soluble agent for plain and reverse spherification. It reacts with sodium alginate to form a skin around the food item.
Animal feeds[edit]Calcium lactate may be added to animal rations as a source of calcium.[20]
Chemistry[edit]The compound was formerly an intermediate in the preparation of lactic acid for food and medical uses. The impure acid from various sources was converted to calcium lactate, purified by crystallization, and then converted back to acid by treatment with sulfuric acid, which precipitated the calcium as calcium sulfate. This method yielded a purer product than would be obtained by distilaltion of the original acid.[8]:p180 Recently ammonium lactate has been used as an alternative to calcium in this process.[10]
Water treatment[edit]Calcium lactate has been considered as a coagulant for removing suspended solids from water, as a renewable, non-toxic, and biodegradable alternative to aluminum chlorideAlCl
3.[21]
Addition of calcium lactate substantially increases the compressive strength and reduces water permeability of bioconcrete, by enabling bactria such as Enterococcus faecalis and Bacillus cohnii to produce more calcite.[22][23]

Magnesium citrate is a magnesium preparation in salt form with citric acid in a 1:1 ratio (1 magnesium atom per citrate molecule). The name "magnesium citrate" is ambiguous and sometimes may refer to other salts such as trimagnesium citrate which has a magnesium:citrate ratio of 3:2.
Magnesium citrate is used medicinally as a saline laxative and to completely empty the bowel prior to a major surgery or colonoscopy. It is available without a prescription, both as a generic and under various brand names including Citromag and Citroma. It is also used in the pill form as a magnesium dietary supplement. It contains 11.23% magnesium by weight. Compared to trimagnesium citrate, it is much more water-soluble, less alkaline, and contains less magnesium.
As a food additive, magnesium citrate is used to regulate acidity and is known as E number E345.

In many individuals, bioavailability of calcium citrate is found to be equal to that of the cheaper calcium carbonate.[3] However, alterations to the digestive tract may change how calcium is digested and absorbed. Unlike calcium carbonate, which is basic and neutralizes stomach acid, calcium citrate has no effect on stomach acid.[4] Calcium carbonate is harder to digest than calcium citrate,[4]and calcium carbonate carries a risk of "acid rebound" (the stomach overcompensates by producing more acid),[4] so individuals who are sensitive to antacids or who have difficulty producing adequate stomach acid may choose calcium citrate over calcium carbonate for supplementation. According to recent research into calcium absorption after gastric bypass surgery,[5] calcium citrate may have improved bioavailability over calcium carbonate in Roux-en-Y gastric bypass patients who are taking calcium citrate as a dietary supplement after surgery. This is mainly due to the changes related to where calcium absorption occurs in the digestive tract of these individuals.
